DEADLINE: 2/17/2026

If you received a notice by mail or email, you are automatically included in the class and do not need to submit a Proof of Claim and Release Form.  You are encouraged to use the Unique Identifier and PIN number provided on your notice to verify your name, address and work information is correct.

If your notice states in the “How Can I Get a Payment” section that you must complete IRS Form W-9, please use the Unique ID and PIN provided in your notice to complete and submit IRS Form W-9 for tax reporting purposes. This action can be completed as part of the update process noted above on the Settlement website. If you fall into this category, you must complete and submit IRS Form W-9 in order to receive a payment. You must do so by February 17, 2026.
