If you received a notice by mail or email, you are automatically included in the class and do not need to submit a Proof of Claim and Release Form. You are encouraged to use the Unique Identifier and PIN number provided on your notice to verify your name, address and work information is correct. If your notice states in the “How Can I Get a Payment” section that you must complete IRS Form W-9, please use the Unique ID and PIN provided in your notice to complete and submit IRS Form W-9 for tax reporting purposes. This action can be completed as part of the update process noted above on the Settlement website. If you fall into this category, you must complete and submit IRS Form W-9 in order to receive a payment. You must do so by February 17, 2026. If you did not receive notice in the mail or by email but believe you are a member of the Settlement Class, completing a Proof of Claim and Release Form is the only way you can get money. You must timely submit a valid Proof of Claim and Release Form. Your Proof of Claim and Release Form must be submitted online or postmarked no later than February 17, 2026. See FAQ 13 for more information. Give up your rights to sue Geisinger and Evangelical about the claims in this lawsuit. |
